Key Responsibilities
As a Social Worker in our team, you will play a crucial role in supporting adults with mental health challenges. Your duties will include:
Assessment & Case Management
- Conducting assessments, support planning, and reviews under the Care Act 2014.
- Coordinating care under the Care Programme Approach (CPA).
- Completing and updating risk assessments to ensure the safety and well-being of service users.
Legal & Ethical Practice
- Working within the frameworks of the Care Act 2014, Mental Health Act (1983), and Mental Capacity Act (2005).
- Preparing high-quality reports for legal processes such as Mental Health Tribunals and Hospital Managers Hearings.
- Leading and participating in safeguarding inquiries in compliance with Section 42 of the Care Act.
Multidisciplinary Collaboration
- Working closely with psychiatrists, nurses, occupational therapists, and other health professionals to provide integrated care.
- Demonstrating anti-discriminatory practice and promoting the rights of adults with care and support needs.
- Engaging with service users and their families to develop person-centered care plans.
Professional Development
- Access to regular supervision by an Advanced Social Work Practitioner.
- Support to complete the Assessed and Supported Year in Employment (ASYE) program for newly qualified social workers.
- Training opportunities to qualify as a Best Interests Assessor or Practice Educator.
What We’re Looking For
We seek passionate and skilled professionals with the following qualifications and experience:
Essential Qualifications & Skills
- Recognized Social Work qualification (Degree, CQSW, CSS, Dip SW, or equivalent).
- Registration with Social Work England.
- Understanding of mental health legislation, safeguarding frameworks, and person-centered care principles.
- Strong communication, organizational, and report-writing skills.
Desirable Qualifications & Experience
- Approved Mental Health Professional (AMHP) status or willingness to undertake training.
- Previous experience as a Care Coordinator in community mental health services.
- Knowledge of welfare rights and social care funding legislation.
- Experience in commissioning care packages under the Care Act 2014 and Section 117 of the Mental Health Act.
