Key Responsibilities

As a Social Worker within the Cromwell House Adult Community Mental Health Team, you will play a crucial role in the assessment, planning, and support of adults facing mental health challenges. Your main duties will include:

Assessment & Support Planning

  • Conduct thorough assessments of individuals with mental health needs and develop comprehensive support plans in line with the Care Act 2014.
  • Work closely with service users to ensure their voices are heard and their needs are met in a person-centered approach.

Care Coordination & Risk Assessment

  • Coordinate care plans, ensuring that appropriate services are provided.
  • Complete and update risk assessments to address any safety concerns for clients.

Multi-Disciplinary Teamwork

  • Collaborate effectively with colleagues across different disciplines, ensuring a holistic approach to care.
  • Participate in multi-disciplinary meetings to discuss client progress and treatment.

Safeguarding & Legal Compliance

  • Ensure all assessments, reviews, and actions are conducted in accordance with relevant legislation, including the Mental Health Act 1983, Mental Capacity Act 2005, and Care Act 2014.
  • Contribute to safeguarding procedures, ensuring that the welfare of service users is paramount.

Reporting & Legal Documentation

  • Maintain accurate and timely reports, including those required for Mental Health Tribunals and Hospital Managers Hearings.
  • Develop reports that are legally sound and align with the required legal frameworks.

What We Are Looking For

We are seeking a qualified Social Worker with experience in mental health and a strong commitment to high professional standards. The ideal candidate will possess the following:

Qualifications & Registration

  • Essential: A recognized Social Work qualification (e.g., Degree in Social Work, CQSW, CSS, DipSW, or equivalent).
  • Essential: Current registration with Social Work England.
  • Desirable: Approved Mental Health Professional (AMHP) status or willingness to undertake AMHP training.
  • Desirable: Additional qualifications such as Best Interest Assessor or other post-qualifying courses.

Experience & Skills

  • Essential: Proven experience working with individuals who have mental health needs, including the ability to develop effective relationships with service users and their families.
  • Essential: Strong knowledge of Care Act 2014, Mental Health Act 1983, Mental Capacity Act 2005, and other relevant legislation.
  • Desirable: Previous experience as a care coordinator in community mental health services or experience in conducting adult safeguarding enquiries.
  • Essential: Ability to work autonomously while managing a complex caseload and maintaining professional standards.
  • Essential: Strong communication and organizational skills, with a proven ability to write comprehensive, legally compliant reports.
