Job Summary

Join the Essex Partnership University NHS Foundation Trust (EPUT) as a Community Social Worker. This role is essential to the Crisis Resolution Home Treatment Service, where you’ll work closely with a multidisciplinary team to assess, plan, implement, and evaluate care programs for adults and older adults experiencing mental health issues. You will be directly involved in helping those in crisis by providing tailored, compassionate care in the community.


Key Responsibilities

1. Holistic Care and Client Support

  • Conduct comprehensive assessments to identify client needs and care requirements.

  • Plan, implement, and review individualized care plans for clients with mental health challenges.

  • Ensure accurate documentation of all client interactions and care processes.

2. Collaboration with Multidisciplinary Team

  • Work closely with a team of healthcare professionals to ensure holistic, integrated care for each client.

  • Engage in regular team and clinical meetings, contributing to discussions on new care models and practices.

  • Establish strong communication links with Primary Health Care professionals and ward teams.

3. Risk Management and Client Monitoring

  • Assess and monitor risks to clients’ safety, as well as potential risks to others.

  • Implement risk management strategies and review client progress.

  • Be vigilant in identifying clients with behavioral challenges, such as violent tendencies, and take necessary precautions during visits.

4. Therapeutic Relationship Building

  • Develop a strong therapeutic relationship with clients and their families, ensuring emotional and social support.

  • Provide guidance to clients and families regarding care transitions, including hospital admissions and discharges.

5. Clinical Leadership and Development

  • Lead and contribute to initiatives that aim to improve service quality.

  • Participate in the on-call arrangements for the Crisis Resolution Home Treatment Service.

  • Identify areas for service improvement and collaborate with team members to drive positive change.

Person Specification

Essential Qualifications and Skills

  • Professional Qualification: Must hold a qualification in Social Work, Occupational Therapy, or Registered Mental Health Nurse (RMN).

  • Experience: Experience in mental health services, ideally with adults and older adults experiencing mental health crises.

  • Risk Management: Proficient in conducting clinical risk assessments and managing ongoing monitoring of clients’ mental states.

  • Effective Communication: Ability to communicate clearly and effectively with clients, families, and healthcare professionals.

Desirable Qualifications and Experience

  • Additional Professional Qualifications: Dementia-related qualifications are beneficial.
