An exciting opportunity is now available for experienced Social Workers to join the newly developed Hospital Discharge Team and Community Home First Hub Service at the Northern Care Alliance NHS Foundation Trust. This hybrid role provides support to patients in both the acute setting and those placed in discharge-to-assess and long-term care facilities.
As part of a multidisciplinary team, you will work alongside therapy, nursing, and reablement teams, all focused on delivering personalized, holistic assessments to ensure patients are discharged safely and without delay.

Salary : £34,834 to £41,418 a year (dependent on level offered) per annum
Key Responsibilities
As a Social Worker within the Hospital Discharge Team and Community Home First Hub Service, your duties will include:
- Person-Centered Care: Promote choice and control for patients throughout their journey in both acute and community settings.
- Strength-Based Assessments: Conduct assessments to enhance patient independence and well-being, in compliance with the Care Act 2014.
- Interdisciplinary Collaboration: Work with a variety of professionals, including therapists, nurses, and reablement teams, to ensure effective care and patient support.
- Safeguarding: Apply knowledge of safeguarding processes and conduct mental capacity assessments for patients unable to make specific decisions.
- Guidance and Support: Mentor and support colleagues from other disciplines, especially those with less experience in social care processes.
- Compliance: Ensure that all care and support services comply with relevant legislation, including the Deprivation of Liberty Safeguards (DoLS).
You will be part of a dedicated practice development team offering continuous support through training, shadowing, CPD funding, and access to professional development courses.
Qualifications and Experience
Essential Qualifications:
- Recognized Social Work Qualification (e.g., SW Degree, CQSW, CSS, DipSW, or equivalent)
- Social Work England Registration
Required Experience:
- Demonstrable knowledge of Care Act 2014, Mental Capacity Act 2005, and related legislation.
- Proven ability to develop effective working relationships with service users, carers, and colleagues.
- Strong record-keeping skills, with the ability to communicate complex information clearly in both written and verbal forms.
- Experience in a multi-disciplinary setting, with a strong understanding of the social worker’s role within that context.
- Safeguarding experience and familiarity with Best Interest Assessments.
Desirable Qualifications:
- Additional certifications (e.g., Best Interest Assessor, AMHP, Practice Educator).
- Post-qualifying courses or professional development qualifications.
- Innovative approach to supporting individuals in the community.
Knowledge:
- Understanding of strength-based and person-centered approaches to health and social care.
- Familiarity with integration agendas and enablement models in care settings.
- Knowledge of Welfare Rights Legislation (desirable).
