This team plays a vital role in supporting hospital discharges by delivering short-term and interim care to individuals in their own homes.
Your base will be at Babington Hospital in Belper, covering the localities of Amber Valley and Erewash.

Key Responsibilities
- Supervise and guide Support Workers within the Community Response Team.
- Assign patient visits to ensure effective and timely service delivery.
- Collaborate with Nurses and Therapists to assess and treat patients, progressing intervention programs as necessary.
- Communicate effectively with patients and carers to support rehabilitation goals.
- Act as a role model, maintaining high standards of clinical skills and professionalism.

Qualifications and Experience
Essential Requirements:
- NVQ Level 4 or equivalent knowledge gained through training and experience.
- GCSE in English and Maths.
- Previous experience as a Band 3 Support Worker.
- Community care experience, particularly with adults or older people in a rehabilitation setting.
- Full UK driving license and access to a reliable vehicle.
Desirable Skills:
- Strong manual handling skills.
- Ability to work flexibly to meet the needs of the service.
- Experience working within a wide geographical area.
