Based in central Hereford, this team collaborates with Worcestershire CLD teams, GP surgeries, and other primary and secondary care services to deliver exceptional care to individuals with learning disabilities.

Key Responsibilities
As a Community Healthcare Assistant, you will:
- Deliver person-centered nursing support to individuals with learning disabilities.
- Facilitate access to healthcare services, including medical and specialist appointments.
- Assess and monitor service users’ health and support needs in collaboration with clinical staff.
- Provide training and guidance to service users and carers on health-related interventions.
- Maintain accurate records and contribute to care planning with the clinical team.
- Respond effectively to emergencies and challenging situations as a lone worker.
- Promote service users’ independence and advocate for their needs.
What We’re Looking For
We are searching for someone with:
- Essential Qualifications:
- Level 3 qualification in Health Care (e.g., NVQ3, A-level equivalent).
- Commitment to completing the Care Certificate within 12 weeks of employment.
- Experience:
- At least 3 years’ experience working with adults with learning disabilities.
- Familiarity with personal care tasks and one-on-one support.
- Skills:
- Excellent communication and interpersonal skills.
- Advocacy, observation, and record-keeping abilities.
- Flexibility to work independently and adapt to changing situations.
- Preferred Experience:
- Working in the NHS or a similar public organization.
- Community-based work with learning disabilities.
