  • Position: Community Care Worker
  • Location: United Kingdom
  • Job Type: Full-Time
  • Salary: £29,970 to £36,483 per annum (pro-rata for part-time)
  • Hours: 25 hours per week
  • Band: 5 (Permanent Role)

Key Responsibilities

  • Provide quality advice to individuals and their carers, helping them achieve independence.
  • Conduct holistic assessments under the Care Act 2014, including risk management.
  • Develop outcome-based support plans and personal budgets.
  • Collaborate with third-sector services to meet identified outcomes.
  • Address safeguarding concerns and ensure the safety of individuals.
  • Assist in emergency or crisis situations, helping individuals maintain independent living.
  • Deliver accurate data input and maintain records using electronic systems.
  • Work in partnership with agencies like the NHS and police to address vulnerable individuals’ needs.

Person Specification

Essential Qualifications & Experience:

  • Minimum 1 year of experience in mental health or social care settings.
  • Vocational Level 3 (NVQ 3) in a relevant subject or equivalent qualifications.
  • Proficiency in MS Office applications like Teams, Word, and Excel.
  • Strong interpersonal, planning, and organizational skills.
  • Knowledge of safeguarding principles, risk management, and relevant legislation (e.g., Care Act 2014, Mental Capacity Act 2005).

Desirable Qualifications & Experience:

  • NVQ Level 4 in Health & Social Care.
  • Full driving license.
  • Awareness of multi-disciplinary assessments and NHS Continuing Health Care policies.

Working Conditions

  • Indoor and occasional outdoor work environments.
  • Exposure to challenging situations, including unclean homes and aggressive behaviors.
  • Daily involvement with vulnerable individuals experiencing significant difficulties.
