Key Responsibilities
- Provide quality advice to individuals and their carers, helping them achieve independence.
- Conduct holistic assessments under the Care Act 2014, including risk management.
- Develop outcome-based support plans and personal budgets.
- Collaborate with third-sector services to meet identified outcomes.
- Address safeguarding concerns and ensure the safety of individuals.
- Assist in emergency or crisis situations, helping individuals maintain independent living.
- Deliver accurate data input and maintain records using electronic systems.
- Work in partnership with agencies like the NHS and police to address vulnerable individuals’ needs.

Person Specification
Essential Qualifications & Experience:
- Minimum 1 year of experience in mental health or social care settings.
- Vocational Level 3 (NVQ 3) in a relevant subject or equivalent qualifications.
- Proficiency in MS Office applications like Teams, Word, and Excel.
- Strong interpersonal, planning, and organizational skills.
- Knowledge of safeguarding principles, risk management, and relevant legislation (e.g., Care Act 2014, Mental Capacity Act 2005).
Desirable Qualifications & Experience:
- NVQ Level 4 in Health & Social Care.
- Full driving license.
- Awareness of multi-disciplinary assessments and NHS Continuing Health Care policies.
