Location and Flexibility
The position is based in the United Kingdom, and you can work either full-time, part-time, or on a permanent contract, depending on your preference. The role offers a competitive hourly wage of £11.90 per hour with the potential for career development.
Purpose of the Job
As a Social Care Worker, your core responsibility will be to provide front-line support to individuals, carers, and families, helping them maintain or improve their level of independence. You will collaborate with a multi-disciplinary team to deliver person-centered care, offering a range of personal and practical interventions while adhering to legislative and regulatory frameworks.
Key Responsibilities
Front-Line Support & Care Delivery
- Provide person-centered support to individuals, ensuring that their unique needs are met and that any safeguarding or risk concerns are promptly addressed.
- Assist individuals with personal care tasks, daily living activities, and other practical support based on their individual care plans.
Risk Management & Safeguarding
- Identify and manage risks associated with the care of service users, escalating issues as needed to the Registered Manager for further assessment.
- Ensure that any risks or concerns are communicated in a professional and timely manner, and that appropriate actions are taken.
Collaboration with Families and Carers
- Offer advice, guidance, and information to service users, their families, and carers to ensure that their needs are met in a sensitive and timely manner.
- Collaborate with a multi-agency team to conduct assessments, formulate care plans, and deliver tailored interventions.
Continuous Support and Development
- Implement health plans and support plans that focus on the specific needs of each individual.
- Develop and deliver session plans based on assessed needs and continually monitor progress to adjust care plans as required.
Specialized Care Delivery
- Use specialized health and social care methods to respond to individual needs and enhance care delivery.
- Make the most of community assets to provide holistic responses to service users’ needs.
Qualifications and Experience
Essential Requirements
- 1+ year of experience in home care or a similar role.
- Full UK Driving Licence required due to the nature of the work, which involves travel to various settings.
- Practical experience gained through training and on-the-job work in a social care environment.
- Understanding of relevant regulations and standards in the health and social care sector.
Skills and Abilities
- Strong interpersonal skills, with the ability to build trusting relationships with service users, families, and colleagues.
- Ability to manage day-to-day challenges and deliver high-quality care under pressure.
- Problem-solving skills, with the ability to recognize issues, assess risks, and escalate as necessary.
- Ability to interpret and analyze behavior to make specialist interventions for improved outcomes.
- Proactive approach to embracing innovation and ensuring best practices in all areas of care.
Desirable Attributes
- Knowledge of person-centered care and the ability to tailor interventions to individual needs.
- Experience in working with diverse populations, including individuals from minority or disadvantaged groups.
- Multilingual skills and understanding of different cultural needs in the context of care.
