About the Role
We are seeking a dedicated Advanced Healthcare Assistant to join our Community Nursing Team in Maidstone. This role involves providing compassionate and tailored care to patients in their homes or residential settings, focusing on those with complex or long-term health conditions.
As part of our ‘Outstanding’-rated team by the CQC, you’ll play a critical role in enabling patients to remain safe, well, and comfortable in their chosen environments.

Salary: £24,071 – £25,674 per year (pro-rata)
Key Responsibilities
- Delivering personalized care plans in collaboration with nursing teams.
- Monitoring patient well-being and reporting any changes to the team.
- Supporting patients with diverse needs, including the elderly, terminally ill, or those recently discharged from hospitals.
- Working Monday to Friday (8 AM – 4 PM) with rotational weekend shifts (1 in 4 weekends).

Qualifications and Skills
Essential:
- Relevant Level 3 qualification (NVQ/QCF Diploma or equivalent).
- Functional Skills Level 2 in English and Maths (or equivalent).
- Minimum of 2 years’ experience in healthcare or social care.
- Sound communication and interpersonal skills.
- Valid UK driving license and access to a car.
Desirable:
- Experience in community nursing environments.
- Knowledge of current NHS developments and clinical practices.
