About the Role: Join the D2A Pathway 2 Service
The successful applicant will work within a fast-paced, multidisciplinary team supporting individuals recovering in a rehab-focused, bed-based environment after an acute hospital stay. The role focuses primarily on Pathway 2 of the D2A framework, requiring strong knowledge of statutory assessments, Mental Capacity legislation, and Continuing Health Care protocols.
Key Responsibilities
Delivering High-Quality Social Work Support
- Conduct statutory assessments in complex cases
- Support recovery goals within short-term (up to 12-week) placements
- Maintain effective discharge planning with a focus on client independence
Multidisciplinary Collaboration
- Collaborate with nurses, physiotherapists, OTs, discharge facilitators, and support staff at Haywood Hospital and local care homes
- Coordinate with external stakeholders like Integrated Care Boards (ICBs) and Staffordshire County Council
Timely Case Management
- Prioritize tasks effectively to manage multiple short-term cases
- Ensure safe and person-centered discharges
Professional Development
- Participate in the Social Work Learning Academy (SWLA) aligned with BASW standards
- Pursue continuous professional development and achieve Band 6 capability post-ASYE
Candidate Profile
Essential Qualifications & Skills
- Professional Social Work qualification (DipSW, CQSW, CSS, BA, MA, or approved equivalent)
- Active registration with a UK-recognized Social Work regulatory body
- Excellent IT proficiency (Word, Excel, PowerPoint, Email)
- Commitment to anti-discriminatory practices
- Ability to handle complex assessments and multi-agency coordination
Desirable Experience
- Prior experience with older adults, mental health needs, and their carers
- Exposure to diverse practice areas: children and families, learning disabilities, mental health
